Definition

Aortic incompetence is a valvular heart disease where the aortic valve does not close completely during diastole, causing blood to flow backward from the aorta into the left ventricle.

Aetiology (Causes)

- Congenital bicuspid aortic valve

- Rheumatic heart disease

- Infective endocarditis

- Aortic root dilation (e.g., Marfan syndrome)

- Hypertension

- Trauma or valve degeneration

Clinical Features & Symptoms

- Dyspnea (especially on exertion)

- Palpitations

- Angina

- Fatigue

- Orthopnea and paroxysmal nocturnal dyspnea

- Bounding pulses and wide pulse pressure

History of Blood Pressure

- Systolic hypertension with low diastolic pressure

- Wide pulse pressure is a hallmark feature

On Examination

- Corrigan’s pulse (water hammer pulse)

- Quincke’s sign (capillary pulsations in nail beds)

- Early diastolic murmur best heard at left sternal border

- Displaced apex beat due to left ventricular hypertrophy

Investigations

- Echocardiography (gold standard for valve assessment)

- Chest X-ray (may show cardiomegaly)

- ECG (left ventricular hypertrophy)

- Cardiac MRI (for aortic root evaluation)

- Blood tests (to rule out infective causes)

Differential Diagnosis

- Mitral regurgitation

- Aortic stenosis

- Pulmonary regurgitation

- Patent ductus arteriosus

Complications

- Heart failure

- Arrhythmias

- Endocarditis

- Sudden cardiac death (rare but possible)

Treatment

Conventional

- Medical management: ACE inhibitors, diuretics, beta-blockers

- Surgical: Aortic valve replacement (AVR) or repair

Homeopathic Remedies

- Crataegus oxyacantha: Improves cardiac tone

- Digitalis: For slow, irregular pulse and heart failure symptoms

- Spongia tosta: For valvular murmurs

- Laurocerasus: For cyanosis and weak pulse

- Adonis vernalis: For heart failure with edema

Note: Homeopathy should be used under guidance of a qualified practitioner.

Integrative Approach

Combining Ayurveda, homeopathy, and modern cardiology can offer holistic care, especially in early stages or for symptom relief. Lifestyle changes, yoga, and dietary modifications play a supportive role.
